Your eligibility for telehealth services is determined by your physical location at the time of your visit — not your home address or state of residence. To receive care through Revitalize Me:
- You must be physically located in a state where our providers hold an active license.
- Location verification is required prior to consultation.
- Payment is contingent on jurisdiction eligibility.
- If a provider is not licensed in your state, services cannot be rendered.
State-Specific Disclosure Requirements
In compliance with state law, Revitalize Me acknowledges the following public disclosure requirements:
- Massachusetts (MA): Provider licensure information is publicly disclosed upon request.
- Rhode Island (RI): Provider licensure information is publicly disclosed upon request.
- Vermont (VT): Provider licensure information is publicly disclosed upon request.
- California (CA): Provider licensure information is publicly disclosed in accordance with state law.
