Provider Information We Make Available
To support informed choice and regulatory transparency, RevitalizeMe Telehealth may make certain provider-identifying information available to patients and the public. This may include provider name, credentials, licensure state(s), and professional biography. Such information is disclosed only where appropriate and in accordance with applicable law.
HIPAA Integration
RevitalizeMe Telehealth and its affiliated providers handle Protected Health Information (PHI) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and applicable state privacy laws. Our platform integrates HIPAA-related practices as follows:
- Patient consent: Patients acknowledge and accept our HIPAA Authorization and telehealth consents during registration and before certain services (e.g., lab orders). These consents are recorded and linked to the patient record.
- Access controls: Access to PHI is restricted to authorized personnel and licensed healthcare professionals involved in patient care. Provider access is limited by state licensure (e.g., providers may only access patients in states where they are licensed).
- Minimum necessary: Use and disclosure of PHI is limited to what is necessary for treatment, payment, healthcare operations, and as permitted or required by law.
- Business associates: Third-party service providers that handle PHI (e.g., lab partners, messaging) are used in a manner consistent with HIPAA requirements and appropriate safeguards.
